By submitting this form, I confirm that our business is committing to the selected sponsorship level(s) and total amount. I understand that payment may be made immediately by credit card or invoiced upon request. If invoiced, payment will be due no later than 90 days prior to the sponsored event date.

I acknowledge that Sidney Alive will begin promoting our business as a sponsor upon receipt of this commitment, and that sponsorships are considered final and non-refundable, except in the case of event cancellation by Sidney Alive.
